At Inspire Health, we participate in the Colorado Immunization Information System.  This means that when we give a vaccine in our clinic, we enter that data into the Colorado Immunization System.  This allows health care providers to have access to your immunization history in the event they need to know your immunization history.  This system is a confidential program within the Colorado Department of Public Health and Environment.  You have the option to opt out of having your vaccine information entered into the Colorado Immunization Information System.  Below you will find FAQs for the Colorado Immunization Information System, as well as notification of your right to opt out.  Should you choose to opt out of having your data entered into the state system, you need to let us know at the time of your vaccination and we will provide you with an opt out form.

What is the Colorado Immunization Information System?

Also known as Colorado’s immunization registry, the Colorado Immunization Information System (CIIS) is a confidential, population-based, computerized system that collects and consolidates vaccination data for Coloradans of all ages from a variety of sources. CIIS is a program within the Colorado Immunization Section at the Colorado Department of Public Health and Environment (CDPHE). CIIS helps healthcare providers, schools, childcare centers, universities, and individuals keep track of the shots that they and/or their children have received.
Why is this important?
The schedule of shots that is necessary for you/your child to stay healthy and required for your child to attend school, daycare, camp and university becomes more complicated with every new vaccine development. Keeping all the shots in one place, and on one record, helps ensure that you and your child get the complete schedule of immunizations – no more and no less.
What information about me/my child will be entered into CIIS?
The shots that you/your child have already received and any shots that you/your child will receive in the future will be entered into CIIS. Other information, including parent’s/guardian’s name (for individuals under 19 years of age), gender, Vaccines For Children eligibility status (for individuals under 19 years of age), date of birth and the office where each shot is given will be included in the registry to be sure that your/your child’s record is accurate and cannot be confused with another patient’s record. Your address will also be included in the system to allow your healthcare provider to send you reminders when you/your child are due for an immunization. All information entered into CIIS is secure and confidential.
How can CIIS help me?
In addition to helping you keep track of your/your child’s shot record, CIIS also:

  • Consolidates information for any shots that you/your child have received from multiple healthcare providers into ONE record

  • Allows your healthcare provider to see what shots are needed to keep you/your child healthy

  • Helps ensure that you/your child do not miss any shots or get unnecessary shots

  • Provides an official copy of your/your child’s immunization history to healthcare providers, schools and childcare centers

  • Helps ensure that your/your child’s immunizations are up-to-date

  • Allows your healthcare provider to remind you when it is time for your/your child’s next shot

  • Keeps records safe during natural disasters or other emergencies

  • Helps prevent disease outbreaks in your community

    Who can receive copies of my/my child’s shot records from CIIS?

    Information in CIIS can only be released to:

  • You (through your healthcare provider or a formal CIIS record request)

  • Your/your child’s healthcare provider or hospital

  • A school, childcare or university where you/your child are enrolled

  • A managed care organization or health insurer where you/your child are enrolled

  • Persons or entities who have an agreement with the State of Colorado for immunizations

  • The Colorado Department of Health Care Policy and Financing if you/your child are enrolled in Medicaid

    Can I get my/my child’s immunization records directly from CIIS?

    Yes. You may request copies of your/your child’s immunization record by completing the Request to Release Immunization Record Form accessed through the Individuals>Request a Shot Record tab on our website: www.ColoradoIIS.com. Forms must be notarized and submitted to CIIS with a legible photocopy of your driver’s license or other secure and verifiable document. Prior to completing the form, please contact CIIS to confirm that the requested record is in CIIS. Please note that CIIS is a voluntary system and, as such, does not contain complete immunization records for ALL Coloradans.

    What if I don’t want my/my child’s shot information entered into CIIS?

    You can choose to have your/your child’s immunization data excluded from CIIS at any time. Please contact your healthcare provider for more information on how to opt-out of the system.
